Cupping creates negative pressure that decompresses soft tissue, improving microcirculation and releasing myofascial adhesions in a way that compressive manual therapy cannot replicate. The temporary petechiae (sha) that sometimes appear are a therapeutic response indicating increased blood flow to the treated area — not bruising. They resolve within 3–7 days and are associated with faster recovery in clinical research.

Clinical gua sha applies controlled stroking pressure using a smooth-edged instrument to break up fascial adhesions, reduce inflammation-driving cytokines, and restore range of motion.
